Field Trip forms are located are located on the SRVUSD website. The password is "forms" in all lowercase. 

FIELD TRIPS (WIthin 100 Miles and NOT Overnight)

 

California High School Field Trips

Category One

(Within 100 Miles- NOT Overnight)

 

  • Read Board Policy 6153: School Sponsored Trips
  • Read School Trip Planning Guide
  • Submit a School Trip Request Form to Dayna Taylor at least one month before the field trip for approval.
  • Busses must be booked through an approved charter bus company. Please see list. Once company is selected please fill in on School Trip Request Form. Dayna Taylor will then submit this form to the Transportation Office for approval.
  • Send home School Trip Permission/Emergency Information & Student Code of Conduct.
  • All drivers (including teachers) must turn in a Personal Automobile Permission Form.  This includes attaching a photocopy of the drivers license and proof of insurance.
  • Trip must have a 15-1 student to adult ratio. Minimum two adults.
  • Work out financial considerations with Ann Weidinger.

 

Three Days Prior to Trip Turn in To Lynette Scharlach:

  • List of student and parent names (class roster preferred)
  • Personal Automobile Use Permission Forms (drivers license & proof of insurance) 
  • Any last minute changes please notify Lynette immediately.

Teacher Keeps:

  • Student Conduct Forms & School Trip Permission Forms

 

All digital forms can be located on : http://www.srvusd.k12.ca.usunder Staff Resources and then in a section labeled SRVUSD Forms.  All information is in a folder calledSchool Trips. The password is “forms” in all lowercase letters.

FIELD TRIPS (Over 100 Miles/Overnight/Out of State)

 

California High School Field Trips

Category Two & Three

                  (Over 100 miles/Overnight/ Out of California)

 

  • Make an appointment with Dayna Taylor to discuss the trip.
  • Read Board Policy 6153: School Sponsored Trips
  • Read School Trip Planning Guide
  • Submit a School Trip Request Form to Dayna Taylor. This form must be typed and printed out through SRVUSD website. Category 2 Trips must be submitted for approval at least three months prior to trip. Category 3 Trips must be submitted at leastsix months prior to trip and nine months if trip requires international travel.
  • Category 2&3 trips require a typed itinerary and short summary and educational value of the trip. This must be submitted with the School Request Form.
  • Category 3trips must type out a Preliminary School Trip Form online.
  • Busses must be booked through an approved charter bus company. Please see list. Once company is selected please fill in on School Trip Request Form. Dayna Taylor will then submit this form to the Transportation Office for approval.
  • Send home School Trip Permission/Emergency Information & Student Code of Conduct.

  • Parents must watch the SRVUSD Volunteer Orientation video on the SRVUSD website: http://www.blueskyvideopro.com/projects/SRVUSD/VolunteerPilot/player.html then print out the Volunteer Orientation Form after viewing. This must be on file before the trip.
  • Confidential Medical Authorization and Medical HistoryForms (If needed)
  • Trip must have a 10-1 student to adult ratio. Minimum two adults. All overnight trips require a mixture of male and female chaperones.
